SUMMARY. Despite the high live birth success rates for couples undergoing in vitro fertilization (IVF), there is still high
share of iatrogenic twin and higher order gestation conceived from this treatment. Twins and multiple pregnancy rate
remains at a level that is considered unacceptable given the associated clinical risks to mothers and babies, and the
additional costs associated with neonatal care of premature and low birth weight babies. The voluntary transfer of a
single high quality embryo,elective single embryo transfer (eSET), has signifi cantly reduced multiple gestation rates and
maximized the rate of singleton pregnancy without compromising overall success rates. The evolution and future
applications of this practice to reduce the risk of iatrogenic twins is reviewed.
